B Cell Basics

B Cell Development Stages

• B cell development occurs in the bone marrow and involves several distinct stages
• Hematopoietic stem cells give rise to common lymphoid progenitors (CLPs), which are the precursors of B cells
• CLPs differentiate into pro-B cells, the earliest stage of B cell development
  • Pro-B cells begin rearranging their immunoglobulin heavy chain genes (VDJ recombination)
• Pro-B cells develop into pre-B cells, which express a pre-B cell receptor (pre-BCR) on their surface
  • Pre-BCR consists of a successfully rearranged heavy chain paired with a surrogate light chain
  • Pre-BCR signaling promotes cell survival and proliferation
• Pre-B cells differentiate into immature B cells, which express a complete BCR (IgM) on their surface
• Immature B cells undergo negative selection to eliminate self-reactive cells and ensure self-tolerance
• Surviving immature B cells migrate to secondary lymphoid organs (spleen, lymph nodes) as mature naive B cells

Antibody Structure and Function

• Antibodies, also known as immunoglobulins (Ig), are Y-shaped proteins produced by B cells
• Consist of four polypeptide chains: two identical heavy chains and two identical light chains
• Heavy and light chains are connected by disulfide bonds, forming a flexible hinge region
• Each antibody has two antigen-binding sites (Fab regions) at the tips of the Y-shaped structure
  • Fab regions contain variable domains that determine antigen specificity
• The stem of the Y-shaped structure is called the Fc region, which mediates effector functions
  • Fc region interacts with complement proteins and Fc receptors on immune cells
• Antibodies have several functions in the immune response:

• In the germinal centers, B cells undergo clonal expansion, producing many identical copies of themselves
• B cells also undergo somatic hypermutation, introducing random mutations in their immunoglobulin genes
  • Somatic hypermutation generates antibodies with higher affinity for the antigen
• B cells with high-affinity antibodies are positively selected and differentiate into plasma cells or memory B cells
• Plasma cells secrete large quantities of antibodies into the bloodstream and other body fluids
• Memory B cells provide long-lasting protection and can quickly respond to future encounters with the same antigen
• Antibody production is regulated by cytokines and interactions with T helper cells
  • T helper cells provide co-stimulatory signals (CD40L) and cytokines (IL-4, IL-21) to support B cell differentiation

B Cell Activation and Differentiation

• B cell activation occurs when a naive B cell encounters its specific antigen
• Antigen binds to the BCR on the B cell surface, triggering intracellular signaling cascades
• B cells can be activated through two main pathways: T cell-dependent (TD) and T cell-independent (TI)
  • TD activation requires help from T helper cells, which provide co-stimulatory signals and cytokines
  • TI activation occurs in response to certain antigens (polysaccharides, lipids) that can directly stimulate B cells
• Activated B cells differentiate into either plasma cells or memory B cells
• Plasma cells are terminally differentiated cells that secrete large quantities of antibodies
  • Plasma cells migrate to the bone marrow and can survive for months or years, providing sustained antibody production
• Memory B cells are long-lived cells that persist after the primary immune response
  • Memory B cells express high-affinity BCRs and can quickly differentiate into plasma cells upon re-exposure to the antigen

Types of Antibodies

  • Patients with XLA are susceptible to recurrent bacterial infections and require lifelong immunoglobulin replacement therapy
• Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by the production of autoantibodies against nuclear antigens

  • Treatment strategies for SLE include B cell depletion (rituximab) and inhibition of B cell survival factors (belimumab)
• Multiple myeloma is a malignancy of plasma cells, characterized by the accumulation of monoclonal antibodies in the blood and urine
  • Caused by genetic and epigenetic alterations in plasma cells, leading to uncontrolled proliferation and survival
  • Treatment options include chemotherapy, proteasome inhibitors (bortezomib), and immunomodulatory drugs (lenalidomide)
• Monoclonal antibodies are widely used as therapeutic agents for cancer, autoimmunity, and infectious diseases
